Morgan & Associates, may contact you to gather feedback on your recent healthcare experience or surgery. In the following weeks, you may be contacted in the following ways:

You may receive an email from patient-experience@patient-feedback.com (make sure to check your spam/junk folder). After you receive the email, you’ll need to click on the link provided and input the access code and date of birth located within the email to complete and save the survey.

Telephone:

You’ll receive a call from J.L. Morgan (using an Alabama 205 area code) on behalf of our facility to complete a survey. If you do not have an email address, you can still participate in the survey over the phone.
